Raffles Medical Group’s Terms and Conditions

1. Stated rates are in Singapore dollars and inclusive of 9% GST.
2. For GP services:
   1. Consultation rate excludes, practice cost, medicine, any additional treatment, laboratory and diagnostic investigations.
   2. Consultation rate may vary according to time, length of consultation and clinic location. Services are available at Raffles Medical clinics in Singapore and are not applicable at Raffles Executive Medical Centres, Raffles Medical clinics located in Changi Airport Transit areas, and Raffles24 Acute and Critical Care at Raffles Hospital.
3. For dental services:
   1. Stated rate does not include consumables fee, costs of prescribed medications, injections, X-rays, and other investigations.
   2. Patients will be informed before treatment if any fee to be charged differs from the rate stated above under the Privileges for the OCBC SeniorCare.
   3. Unutilised items and/or services in the package are non-refundable or exchangeable.
   4. A consumables fee of $19.62 is applicable for each dental visit.
   5. Services are available at Raffles Dental (GP) clinics in Singapore.
4. For health screening:
   1. Prior appointment is required.
   2. Unutilised items and/or services in the packages will not be refunded or replaced.
   3. Services are available at Raffles Medical GP clinics in Singapore.
5. For LPA: Applicants are required to apply for and draft their LPA online at the Office of the Public Guardian (Ministry of Social and Family Development) website first, before making an appointment with a Raffles Medical doctor, who will act as an LPA certificate issuer. 6. Prior appointment is required for health screening, LPA certificate issuance and dental services.
7. Other corporate and insurance programmes are not applicable with the OCBC SeniorCare.
8. Listed rates, terms and services are subject to change without prior notice.

General Terms and Conditions

1. Payment must be made with an OCBC Card.
2. An OCBC Card refers to any Visa, MasterCard credit/debit card issued by OCBC (whether or not it also bears the name and/or mark of any other person or entities).

Voucher redemption

• This OCBC SeniorCare voucher is redeemable with purchase of any whole birthday cake at Swensen’s.
• Not valid for festive cakes. The discounts under this Promotion will not apply for festive cakes, and spending on festive cakes will not go towards calculating the S$65 spending to obtain the 15% off discount off birthday cakes.
• Valid from 29 March 2025 to 31 December 2025 (both days inclusive). Redemption of this voucher is subject to the terms and conditions printed on this voucher.
• Valid for walk-in takeaway cake orders only (not for dine-in, online orders or home delivery). Valid at all Swensen’s Singapore outlets listed at https://swensens.com.sg/singapore/
• This Promotion is valid for voucher-holders born in or earlier than the year 1965, with an original printed OCBC SeniorCare voucher. Valid during the voucher-holder’s birthday month only. Voucher redemption:

• The S$2 OCBC SeniorCare voucher is redeemable with a minimum nett spend of S$20 in a single receipt, and redemption is subject to the terms and conditions printed on the voucher. The minimum spend of S$20 can comprise both house brand items and non-house brand items, provided the receipt includes at least one Royal Golden Mali Premium Fragrant Rice 5kg (“Participating Product”).
• Min. spend is not inclusive of Infant Formula Milk Power (Stage 1 & Stage 2), Tobacco, Alcohol, Medicinal Products & Devices, Disposable Carrier Bag Charge, Phone Cards and Sheng Siong Vouchers. The same receipt qualifying for redemption of this OCBC SeniorCare voucher cannot contain spending on the Excluded Products.
• The voucher can be utilised in-store only, at all Sheng Siong Supermarkets in Singapore from 29 March 2025 to 30 June 2025 (both days inclusive), unless otherwise notified.
• The original printed physical S$2 OCBC SeniorCare voucher must be presented during redemption.
• Present the original printed OCBC SeniorCare voucher to the cashier before payment. Only original printed physical vouchers will be accepted. Screenshots, photocopies, or photos of the voucher will not be valid for redemption.
• Each OCBC SeniorCare voucher contains a unique barcode that is valid for one-time use only. The barcode will be considered used once it is scanned at the cashier even if the checkout is not completed, and cannot be reused.
